Change Order Request Form

This form is to be used to initiate research to put together pricing and scheduling for a change order. Follow these steps:

1) Fill out the form below.

2) Mail or drop by a check for $100 made payable to Superior Construction LLC. In the memo please write “research fee”. Nothing will be processed until the money is received.

3) Upon completion of this form and money received then Superior will put together the pricing and will calculate the time added to construction (if applicable). You will be notified via email within 3 business days of the total cost to complete the change order which will include the $250 change order fee per request form.

4) Customer will then have 3 business days to approve the change order and pay in full. If payment and signature of acceptance is not received with 3 business days then the change order will be voided.”

5) If the change order is accepted then the $100 research fee will be credited back to the customer’s change order costs.
